Transition to/from hospital

 

For residents of aged care facilities, transfer to and from hospital can be complex. Many divisions have adopted the use of a common transfer document – ‘The Yellow Envelope” to improve clinical handover from the residential aged care facility to the hospital.

 

North East Valley Division of General Practice (NEVDGP) is currently the lead agency in a project to implement and review transfer information from an RACF to hospital. The project involves seven divisions of general practice, 28 RACFs and five acute health services in the Melbourne metropolitan region.
