Practice capacity for chronic disease management, early intervention and prevention.
GPV has a strong commitment to supporting divisions and through them, general practice to achieve quality population health outcomes through effective chronic disease management, early intervention & primary prevention.
Many chronic diseases are long-term in nature and are highly prevalent in Australia. In 2001, 78% of respondents in the National Health Survey said they had one or more long-term health conditions. Therefore, people with a chronic disease form a significant proportion of the patients visiting general practice.
General practice manages chronic diseases by providing on-going support for management, counselling, drug prescriptions, and co-ordination of care including referrals to specialists and other relevant services. General practice also helps with prevention of chronic diseases by monitoring the biomedical risk factors and supporting lifestyle change to modify behavioural risk factors. www.aihw.gov.au/cdarf/data_pages/gp_practice/index.cfm
Through building the capacity of divisions to effectively promote and support implementation of planned and systematic chronic disease prevention and management in general practice, GPV aims to increase the ability of Victorian general practice to implement systematic care of their practice populations and realise the clinical and business benefits.
Key Objectives
- To support Victorian divisions of general practice to improve understanding in general practice, of the clinical and business benefits of systematic care of their practice populations.
- To support Victorian divisions of general practice to undertake activities to deliver on National Quality and Performance Indicators related to CDM and prevention.
- To support Victorian divisions of general practice to further increase their capacity to effectively promote systematic health care in general practice.
- To work to ensure developments in the Victorian health system result in an effective interface with divisions of general practice and through them general practice.
- To work to ensure national chronic condition prevention and management initiatives are effectively informed and influenced by the experience and knowledge of Victorian divisions and general practice.
