LAOS Screening Criteria

 

From 1 July 2009

  • Patient Death
  • Unplanned return to theatre within 7 days
  • Unplanned re-admission within 28 days
  • Transfer to another health service
  • Patient’s length of stay greater than 35 days
  • Any record which has been recommended by a doctor or other health professional for review

 

From 1 July 2007 – 30 June 2009

  • Unexpected patient death
  • Unplanned return to theatre within 7 days
  • Unplanned re-admission within 28 days
  • Unexpected transfer to another health service
  • Patient’s length of stay greater than 35 days
  • Any record, which has been recommended by a doctor or other health professional for review

 

Prior to 30 June 2007

  • Patient death
  • Return to theatre within 7 days
  • Unplanned re-admission within 28 days
  • Transfer to another acute care facility
  • Patient length of stay greater than 21 days
  • Unexpected readmission following discharge from another hospital
  • Any record which has been recommended by a general practitioner or other staff for peer review
