
LAOS Screening Criteria
From 1 July 2009
- Patient Death
- Unplanned return to theatre within 7 days
- Unplanned re-admission within 28 days
- Transfer to another health service
- Patient’s length of stay greater than 35 days
- Any record which has been recommended by a doctor or other health professional for review
From 1 July 2007 – 30 June 2009
- Unexpected patient death
- Unplanned return to theatre within 7 days
- Unplanned re-admission within 28 days
- Unexpected transfer to another health service
- Patient’s length of stay greater than 35 days
- Any record, which has been recommended by a doctor or other health professional for review
Prior to 30 June 2007
- Patient death
- Return to theatre within 7 days
- Unplanned re-admission within 28 days
- Transfer to another acute care facility
- Patient length of stay greater than 21 days
- Unexpected readmission following discharge from another hospital
- Any record which has been recommended by a general practitioner or other staff for peer review
